Connections: Connecting Families to Community Resources

The purpose of the Connections program is to get to know parents and their children and quickly introduce them to services in the community that they may want or need. Connections accomplishes this by providing broad, efficient and sensitive screening of multiple risk factors in families for the express purpose of providing those families with timely, appropriate referral support. Connections is a creative approach based on the idea that early identification and timely access to appropriate services leads to healthy development and children entering school ready to succeed.
